A Study for Estimating Nursing Manpower in Hospital

Abstract

As nursing service expands in hospital, the estimation of manpower and it¢¥s appropriate utilization becomes the most important concern to the nursing administrators in order to maintain quality patient care and hospital cost saving.
The purpose of this study is to provide a useful data base for the efficient utilization of nursing staff according to the classification of patients, the analysis of nursing activity and
patient care required.

This study was conducted on the hospitalized patients and nursing personnel in Inje College Paik Hospital from June 30 through August 3, 1986.
The results are as follows:
1. Distribution of Patient by Classification
(1) The average number of inpatients was 473. There was no significant difference in number of patients by weekdays.
(2) The distribution of patients according to the classification of patients is as follows: acutely ill (Class M ), moderately ill (Class N ) and mildly ill (Class I ) patients was 21.0%, 39.7%, and 39.3% in seven nursing units.
(3) The distribution of patients in each Class was different in average number of inpatients (between nursing unit).
2. Analysis of activities of Nursing Personnel¢¥s
(1) Nursing personnel spent 23.8% of their
working time for direct nursing activities
and 70.3% for indirect nursing activities
in medical nursing unit. Whereas nursing
personnels in surgical nursing unit spent 25.7% of their working tine for direct nursing activities and 68.4.¢¥-¢¥o for indiret nursing activities.
(2) The head nurses spent their working time mostly for coordination among departments and nursing round for the patients.
(3) The staff nurses spent most of their working time for intravenous injection in terms of direct nursing activities and for paper work in terms of indirect nursing activities in medical and surgical units.
(4) The nurse aids spent most of their working time for changing the sheet in terms of direct nursing activities and for messengerial work in terms of indirect nursing activities in medical and surgical units.
(5) The nurses on day duty worked on average 9 hours and 10 minutes, on evening duty 8 hours and 20 minutes, anal on night duty 11 hours and 45 minutes.
3. Patient¢¥s Care Requirement.
(1) The average duration of care required per day in. time was 60 minutes for Class 1, 90 minutes for Class 11 and 184 minutes for Class M patients in medical nursing unit. In surgical nursing unit, the average duration of care required per day was 43 minutes for Class I , 68 minutes for Class 11 and 127 minutes for Class Ill patients.
(2) The patients were provided with 70.6% of nursing care required by the nursing personnel in medical unit and 87.7% of nursing care required by the nursing personnel in surgical unit.
(3) The patients -received 29.7% and 12.0% of nursing care required by the family members in medical and surgical units respectively.
